Spend limits

Track usage from gateway reports and apply budgets / hard limits.

Usage history: openbot-platform/usage.jsonl. Budgets: usage-budgets.json. Dashboard RPCs expose hard limit get/set for the frozen Usage UI. Desktop Settings shows spend against the cap; editing may remain on the Usage section.
